This position is funded from the Commonwealth's annual operating budget.Duties:
Assists in the handling, full understanding and monitoring of persons served, Individualized Action Plan, through direct observation and, full knowledge of the persons served history, and contact with the persons served in order to assist he/she in learning the necessary skills to live successfully in the community.
Maintains a working knowledge of all DMH regulations pertaining to Community Based Flexible Support Program and Joint Commission procedures. Reading the house manual and supervision by the CBFS team leader or by designee to ensure compliance and quality care. Works as an advocate to assigned persons served by direct one to one contact to provide that person served with a key person to assist them in all areas and to aid them in the implementation of the Individualized Action Plan. Supervise/Assist/ Educate persons served with ADL's, & program safety issues according to DMH Regulations. Supervises persons served and/or assists in general housekeeping duties in assigned areas. Responsible for knowledge of all medications used within the program including their indications, risks and side effects, knowledge and implementation of client medication procedures in order to teach persons served all aspects of their medications through medication related materials at the program, MAP training and any other available trainings. Is able to Administer medication to persons served as needed as after passing MAP training and becoming MAP certified. Facilitates/provides transportation to ensure appointments are kept. Accompanies persons served to and from appointments as assigned. Provides supervision and documentation of assigned staff. Participates in Individualized Action planning meetings. Provides appropriate documentation in medical record. Provides an oral shift report to at least one member of the oncoming shift by reading and writing all pertinent information in the vital log and communicating with the person in charge all required and necessary information. Provides coverage in all state residential programs as needed. Participates in staff meetings to discuss clinical issues. Monitors individual action plans. Responsible for explanation and implementation of policies and licensing procedures in group home. Responsible for all appropriate record keeping to ensure documentation of persons served progress and to ensure compliance with existing DMH Regulations and program procedures through direct supervision of the program manager or designee. Facilitates assigned group meetings. Maintains and cleans house, van and yard on an ongoing basis as assigned by CBFS Team Leader or designee to ensure that the condition of the house and grounds meets Quality Assurance, Joint Commission, DMH, and CMR regulations, Fire, Health and Safety Regulations. Responsible for assisting Persons served in a teaching atmosphere to educate them in ADL skills such as personal hygiene, cooking housekeeping, clothing maintenance and nutrition by utilizing counseling techniques, role modeling and/or other individualized or group methods to promote individual growth. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related and cultural/spiritual needs of persons served on his/her assigned program/service. Attends mandatory trainings as necessary.
Qualifications:
MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) one year of full-time or equivalent part-time experience in providing direct care service to the mentally ill, emotionally or physically handicapped, or (B) any equivalent combination of the required experience and the substitutions below.
Substitutions:
I. An Associate's degree or higher with a major in psychology, human services, mental health or retardation may be substituted for the required experience.
Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.

Work Schedule: (40 hour position) 11:30am to 8:00pm with Tuesday and Wednesday as days off
